    Health Education Mini-Grant Program

    Since 2005, the North Penn Community Health Foundation has funded the Health Promotion Council to implement the Wellness Initiative for the School Environment: Smart Nutrition and Activity Collaborative (WISE SNAC®). The overarching goal of the WISE SNAC® Initiative is to prevent childhood obesity in the Greater North Penn region by fostering school/community partnerships that promote consistent educational messages and opportunities for healthy eating and physical activity. WISE SNAC® is based upon the Center for Disease Control’s evidence-based Coordinated School Health Program Model (CSHP), which identifies eight areas within the school environment that can be targeted in order to improve students’ health outcomes.

    The North Penn Community Health Foundation Health Education Mini-grant program aligns with the efforts of the Health Promotion Council’s WISE SNAC® Initiative to promote opportunities for students to learn about and engage in healthy eating and physical activity.

    Proposed Projects

    Proposed projects must be student-focused and support the goals of the WISE SNAC® Initiative by addressing nutrition and/or physical activity, and when possible collaborating with community organizations. Proposed projects must be consistent with your school district’s Wellness Policy and Administrative Guidelines/Regulations, and if applicable, align with your school’s School Health Council/Committee goals and objectives.  Applicants are encouraged to propose activities modeled after evidence-based or best practice programs. Mini-grant applicants are required to propose projects that target at least one area of the CSHP Model. A list of promising practices is included in Attachment A of the mini-grant application. Mini-grant projects that promote educational messages must ensure the messages are consistent with the US Dietary Guidelines for Americans, MyPyramid, and US Physical Activity Guidelines for Americans. Proposed projects may target the entire student population at a particular school, or focus on a specific class, grade, or identified at-risk student population.  
     
    Funding Limitations

    The mini-grant program does not fund requests for basic school supplies, textbooks, equipment and supplies used for general purposes, food (unless used as part of programming, such as in food demos), travel expenses for speakers, individual student projects, or faculty professional development activities. Applicants are encouraged to leverage mini-grant funds with other funding sources, such as Educational Foundations and Home & School Associations.

    Request Procedures

    An application may be submitted at any time. Complete the special mini-grant application form and forward it to the Foundation. A funding decision generally is made within three weeks of receipt of your application.

    Eligible Schools

    Schools (both public and private) located within the boundaries of the North Penn, Souderton Area and Wissahickon School Districts are eligible for the Health Education Mini-grant program. Elementary schools are eligible to receive $500 per academic year and secondary schools are eligible to receive $1,000 per academic year.
